Specialist page Seizure first aid

Seizures can be frightening — for the person experiencing them and for their family. Getting the right diagnosis and treatment from a qualified epilepsy specialist is the first step toward seizure control and a better quality of life.

At Gujarat Epilepsy & Neuro Clinic (Elite Magnum, Ahmedabad), our NIMHANS-trained neurologists provide evidence-based seizure treatment tailored to each patient.

Treatment pathway

1

Accurate Diagnosis

Detailed clinical history and examination, EEG testing, MRI review, and clarification of whether episodes are truly epileptic.

2

Personalized Medication Plan

Selection of the most appropriate anti-seizure medication based on seizure type, age, sex, comorbidities, and daily life with careful dose adjustment.

3

Ongoing Monitoring

Regular follow-up, seizure tracking, medication adjustment, safety counselling, and review of breakthrough patterns.

4

Advanced Options

For drug-resistant seizures: Video EEG monitoring, surgery evaluation, and escalation beyond standard medication-only treatment.

When treatment is not working

If seizures continue after trying two appropriate anti-seizure medicines, that raises concern for drug-resistant epilepsy. At that point, the next step is not just repeating the same plan. It usually means reviewing the diagnosis again, checking adherence and seizure type carefully, and considering Video EEG, imaging review, and surgery evaluation when suitable.

Patients who keep having seizures should also review safety issues including seizure first aid, water safety, injury risk, and driving restrictions until seizure control is stable.

What seizure treatment usually includes

Good seizure treatment is not just "start one medicine and wait." The plan usually has several parts working together:

  • confirming whether the events are truly epileptic
  • classifying seizure type correctly
  • choosing the right anti-seizure medicine for that seizure pattern
  • checking whether EEG, MRI review, or Video EEG is needed
  • reviewing sleep, missed doses, alcohol use, fever, and other seizure triggers
  • building a safety plan for home, work, travel, and bathing
  • deciding early when standard treatment is failing and needs escalation

What to expect at the first seizure-treatment visit

Patients often come in expecting only a prescription. A proper first consultation is usually broader than that.

  1. Episode history: what exactly happens before, during, and after the event.
  2. Pattern review: first seizure, repeated seizures, breakthrough seizures, blackouts, staring spells, or unclear attacks.
  3. Test review: past EEG, MRI, blood tests, discharge papers, and older prescriptions.
  4. Treatment decision: whether medicines should start now, be adjusted, or be changed.
  5. Safety counselling: driving, sleep, water safety, injury prevention, and family first-aid advice.

If the diagnosis is still uncertain, the most useful next step may be Video EEG monitoring rather than blind medication escalation.

Why seizures may continue despite treatment

When seizures continue, the answer is not always "stronger medicine." Common reasons include:

Wrong seizure classification
The medicine may not match the seizure type.
Missed doses or side effects
Adherence often slips when the plan is not realistic.
Events that are not epileptic
Some blackout or shaking events need a different diagnosis.
Drug-resistant epilepsy
After two appropriate medicines fail, the case needs structured escalation.

That is why persistent seizures should push the case toward drug-resistant epilepsy review, Video EEG, and in selected patients epilepsy surgery evaluation, instead of endless trial-and-error.

⚠️ Seizure Emergency & First Aid

When to Call 108 (Emergency)

  • Seizure lasting more than 5 minutes
  • Multiple seizures without regaining consciousness
  • First-time seizure
  • Difficulty breathing during or after seizure
  • Seizure occurring in water
  • Injury during seizure
  • Person has diabetes, is pregnant, or has other medical conditions

Seizure First Aid - What to Do

  • Stay calm and stay with the person
  • Turn them on their side (recovery position)
  • Place something soft under their head
  • Remove nearby hazards (sharp objects, furniture)
  • Time the seizure duration
  • Talk calmly and reassuringly

What NOT to Do

  • Do NOT put anything in their mouth
  • Do NOT hold them down or restrain them
  • Do NOT give water, food, or medications during seizure
  • Do NOT leave them alone

Questions patients often ask before booking

Yes. A first seizure still needs proper medical review because the diagnosis may still be uncertain, recurrence risk can vary, and tests like EEG or imaging may change the treatment plan.
Video EEG is considered when routine EEG is not enough, the events are unclear, seizures remain uncontrolled, or a major treatment decision such as surgery evaluation depends on precise classification.
That should trigger structured review for drug-resistant epilepsy, not indefinite trial-and-error. The next step may include medicine review, repeat diagnosis review, Video EEG, and epilepsy surgery evaluation.
